One (1) Employee Safety Card for employees of the Hecla Greens Creek Mining Company as part of their Take five(5) For Safety plan.
Designed to be filled out at the beginning of a shift. The miner reports any comments or concerns related to safety in their work area. Designated areas for work instructions, additional action required, supervisor safety chat, and a checklist of specific items to be checked are located on the front of the card, as well as the name, employee number, date and shift of the employee. On verso is an area to write other concerns or comments on heading and whether it was reported and/or corrected, as well as places for employee, supervisor and other signatures and time of signatures.

Notes:
Addison Field, the then curator of collections, received material from Caroline Cave and Mike Martin during a tour of the Greens Creek Mine in October 2010. Caroline Cave is the health and safety director and Mike Martin is the Mine Supervisor.
